Email meUsually inside one working day, and always inside three. If a week goes by with nothing, assume it went to spam and send it again. That is a real failure mode and it is not your fault.
What you are building, who it is for, and when it needs to be live. If you have a URL, include it. Three lines beats three paragraphs, because the first thing I need to know is whether this fits in four weeks at all.
No. The fee is agreed in writing before we start, so there is nothing to negotiate on the form and nothing for you to guess at. Write anyway if you have a budget in mind, and say so; I would rather hear it early than trade emails about it.
Yes, and I will sign yours rather than send you mine. Say so in the first message and do not send anything sensitive until it is signed. A one line description of the problem is enough to start.

Sometimes, and I will tell you honestly on the first reply. What I do not take is work shaped like a retainer, because one project at a time is the whole reason the four week date holds.
The date moves and I tell you the day it happens, not the week before launch. That is the real risk of hiring one person, and pretending otherwise would be the first thing I got wrong. It is also why there is only ever one start date open, and only one project on it.
Amsterdam and Bangkok, and I work with clients anywhere. It has not mattered so far. Most of the work is asynchronous, and the weekly call is booked at an hour that works where you are rather than where I am. If you need someone in the room, I am the wrong call and I will say so on the first reply.
